Description:

Permanent, supported independent apartments for individuals who are homeless and who have mental illness, who may also have co-occurring substance abuse problems. Supported Housing staff provides in-home and in-community services which teach skills and support the individuals need to live independently. Supported Housing staff ensures the individual’s access to all needed psychiatric, medical, dental and community supports and services to assist their recovery. Independent living residences for disabled adults.

Client Intake hrs: 9:00a-5:00p, Monday-Friday

Intake Process:

Agency referral required. Referral to be made to the Admissions Department of Boley.
Items to bring to intake: Picture ID, Social Security card, Proof of Residency, Proof of Axis I Mental Health diagnosis, any and all historical information.
