Permanent, supported independent apartments for individuals who are Chronically Homeless (HUD Definition) and who have mental illness, who may also have co-occurring substance abuse problems. Supportive housing staff provide in-home and in-community services, skills teaching and supports that the individual needs to live independently. Supportive housing staff ensures the individual's access to all needed psychiatric, medical, dental and community supports and services to assist their recovery. Independent Living residences for disabled adults.

Intake Process:

Referral to be made to Boley's Homeless Referral/Admissions line. Items to bring to intake: Picture ID, Social Security card, Proof of Residency (Pinellas County), Proof of Income (Can be none), Third Party Homeless Verification (Chronic).
